Manan Trivedi, MD
Co-Founder & Pain Management Director
His holistic approach and passion for providing patients with novel, life-changing treatments has helped countless individuals with mood disorders and chronic pain. His expertise has allowed him to safely and effectively treat his patients. This passion helped him form Clear Immediate Care with the goal of providing the community with quality care from real doctors.
Dr. Trivedi is a double board-certified anesthesiologist and pain management physician. He completed his Anesthesiology residency and Pain Management fellowship at the University of Illinois at Chicago. He understands how difficult living with pain can be, and aims to assist his patients in an empathetic and understanding manner.
In his free time, Dr. Trivedi enjoys spending time with his wife and two daughters. He is an avid Chicago sports fan (for better or worse), and loves to travel, play basketball and tennis.
Rinku Shah, DO
Co-Founder & Medical Director
Dr. Shah founded CLEAR Immediate Care with the intention of providing the community with great medicine. Medicine that focuses on prevention of disorders, as well as providing patients with a multidisciplinary support system. She truly enjoys engaging with her patients, and formulating relationships that last; a rarity in today’s medical world. She spends time to build rapport with patients and establish trust in her care.
She is board certified in Family Medicine. Her goal is to help treat the mind and body. She has been practicing for 5 years, and specializes in ADHD and other mental health disorders. She earned her medical degree from Nova Southeastern University, and completed her training at Advocate Lutheran General.
In her spare time, she cherishes being a mom to two little girls and loves navigating the world with her husband.
Nicole Woodward, PA-C
Nicole Woodward is a board-certified physician assistant with a background in Emergency Medicine where she found her passion for providing compassionate, comprehensive medical care to patients. She completed her undergraduate studies at Drake University, earning a Bachelor of Science in Clinical and Applied Health Sciences. While an undergraduate student, she also distinguished herself as a division I athlete on the women’s basketball team. Following her undergraduate education, Nicole pursued her master’s degree at the University of Iowa, recognized as one of the leading PA programs in the nation. Her professional experience is complemented by her work ethic, strong commitment to patient care, and teamwork within healthcare settings.
Outside of her medical career, Nicole is an enthusiastic sports fan, passionately supporting the Iowa Hawkeyes and Drake Bulldogs. She enjoys traveling, staying active, and cherishing time spent with family and friends.
Marisol Munoz, APRN
Marisol Munoz is a board-certified Family Nurse Practitioner. Her background includes emergency and preventative medicine. Working many years in the emergency department brought her to seek her passion, preventative medicine. Marisol was eager to focus her career on preventing the community from seeking primary care in the ER. In her latest experience, she focuses on screening and tobacco cessation. She continues to advance her career by pursuing her doctorate in nursing practice.
Alexx Puckett, PA-C
Alexx Puckett is a board certified physician assistant with experience in Emergency Medicine, Urgent Care and Occupational Health. She completed her studies at Long Island University in downtown Brooklyn rotating through some of the country’s finest hospitals. She has been practicing in different EDs and Urgent cares between Indiana and Illinois for the past 5 years.
Prior to her life as a Physician Assistant, Alexx got her bachelor’s degree in Exercise and Sport Science at Coastal Carolina University. She was also a member of the women’s basketball team which has a division 1 program. In her free time, Alexx enjoys being a mother to her daughter, traveling, spending quality time with family/friends and staying active in the gym.
Sydney Elgomayel, PA
Sydney ElGomayel is originally from Atlanta, Georgia. She graduated from the University of Georgia with a Bachelors of Science in Health Promotion. She then received her Master’s degree at Mercer University in Atlanta. She enjoys being a PA because she gets to connect with her patients and work with them to improve their overall health.
She is a huge Georgia football fan and loves spending time outside and with friends, family, and her dog.
Marteen Barni, FNP
Marteen Barni is a board-certified Family Nurse Practitioner originally from St. Louis, Missouri. She received her Bachelors in Nursing from the University of Missouri and went onto work 9 years in bedside nursing primarily in cardiac and medical surgical intensive care. She received her Doctorate in Nursing from Rush University here in Chicago. Marteen is eager to begin her advanced practice career and help the Chicago community in exceptional patient care.
Outside of her professional life, Marteen enjoys walking along the lakefront, catching the latest Netflix original, and spending time with friends and family.
Puja Patel, PA-C
Puja Patel is a board certified physician assistant with a background in community health. She received her Master’s degree from Rocky VIa University. Puja enjoys building a relationship with her patients and focusing on preventive care.
In her free time, Puja enjoys spending time with her family and friends, traveling, hiking, reading and boxing.
Brianna Fiala, PA-C
Brianna Fiala is a certified Physician Assistant from Nashville, TN. She completed her studies at Northwestern University Feinberg School of Medicine. She primarily has practiced in inpatient medicine, but through this experience, became interested in primary and preventative care. Her favorite part about being a physician assistant is being at the bedside with patients and taking the time to listen and understand the complexities of health problems. She was a nurse for five years prior to practicing as a provider, so always finds herself providing a listening ear to patients. In her free time she likes to spend time with her Basset Hound, Copper, and walk the lakeshore path to stay active.
Ally Daugherty, FNP-C
Alyssa “Ally” Daugherty is a board certified family nurse practitioner with a background in urgent care, weight loss, and emergency medicine. She graduated from Brigham Young University-Idaho in 2011 with her RN and from West Virginia University in 2022 with her Masters in Nursing. Most of her career has been in emergency medicine where she learned the importance of preventing illnesses before they lead to serious complications. She enjoys counseling patients on nutrition and exercise as well as other healthy lifestyle changes.
Outside of her career she is a mother to three children and loves to get on her bike and explore Chicago.
Clayton Kline, PA-C
Clayton is a dedicated Physician Assistant with experience in Emergency Medicine and Neurosurgery. Known for his calm demeanor and sharp clinical skills, Clayton thrives in high-pressure environments where quick decision-making can make all the difference.
Outside of the hospital, he enjoys being outdoors and likes hiking, camping, and exploring new places with his loving girlfriend and their adventurous dog, Winnie. Clayton also enjoys playing video games while keeping up with friends and family out of state. A lifelong Chicago sports fan, Clayton rarely misses a Bears or Cubs game and brings that same passion and teamwork to his medical practice. Whether taking care of patients or on the trail, he’s always ready for the next challenge.
